I have searched but was unable to find answers to my questions.  I will be doing my first Form 1 SBR and have a few questions.

In block 4a, do I put the original manufacturer or me since I am "making it".

In Block 4c, I plan to have both .223 and 6.8 uppers for this lower.  Do I put both calibers?

Same for block 4e/f, I was going to have one size in .223 and possibly a different size in 6.8.

What do I do in block 4h/i?

Maybe I am trying to make this more complicated than it should be???  Thanks for the help.  

If there is a sticky on filling this thing out please point me to it.

If you are starting with a preexisting firearm (reciever), you write in the manufacturers information ie. Stag Arms, New Britain, CT

List one caliber (unless your building an AOW drilling). You can list the other upper (caliber, bbl, OAL) info under additional information

Anu additional information (like markings (import stamps for example), bbl lengths, calibers, OALs, etc... go into block 4h.
Answer 4i honestly. The most commonly given answer, and my favorite answer is "All lawful purposes". Generally, as long as your rationale for wanting the weapon isn't illegal, youi'll be approved. One poster, for example got approved despite writing "Because Chicks dig short barrels". If your state has specific verbiage requirements (like the R&D requirement in NC), you must write that, and actually intend on using the firearm for those purposes.
Maybe I am trying to make this more complicated than it should be???  Thanks for the help.

Thanks, that helps tons.  All my Form 4's were easy as I had a dealer to tell me what to do.  I guess I better hunt down the sheriff for a signature.
