Looks good to me.

My first form 1 that I send in I didn't sign box #10 as trustee.  It still came back signed and approved.

True but I figured since it was a tust and it says name and title, that is my title for the trust.

One of the things you need is a copy of Quicken Willmaker 2007.  If you are an amazon shopper, there are a few versions of the 2007 software.  By reading the amazon reviews, I learned that there is a separate willmaker being sold that includes the book and is a few bucks cheaper.

It does not hurt to check out your state requirements.  Some states I have heard require trusts to be filed with the court.  So I checked the yellow pages and called 2 trust attorneys (they have more listings than escort services..., jk) and a paralegal and they all told me, free of charge, that for my state you only have to file the trust if you are including deeded property such as land.  Otherwise, make sure you notarize it.  Willmaker creates a separate page for notarization.
