I always use titleII.com The documents are a PDF file. You can fill them in with your computer and print them out all nice and neat. You need a form 1 for an SBR. You will also need that form where you attest to the fact that you are indeed a US citizen or not, which is also on the same link. |

The offical forms all have the correct addresses at the top of them. Form 1 sent to WV address and Form 4 to Atlanta. The forms on TitleII have n ot been updated, I checked last night. Forms still give Washington DC and Chicago IL as mailing address for forms which is incorrect. It probably does not matter what the form says at the top as long as you send it to the right place. If you right click and save as from the ATF websight you can download the forms and you can actually type the info into the forms just like the ones frm Title II. ATF seems to have made it into the 21st century in that regard.
