(2) Two Form 4's with C3 dealers info, my info and one passport photo attached to each form.
(1) One Certificate of compliance
(1) One check for $200 made out to the BATFE
(?) FD-258 fingerprint cards

Do I get the CLEO to sign off on both Form 4's ? Is there anything I'm missing ?

CLEO signs both forms. You need 2 sets of fingerprints. Make sure you fill in all the info on the fingerprint cards. Have your dealer circle the "I do" piece so the ATF will discuss status with you.
