Yes, that is the form. When I did mine, I included a copy of my current FFL which I signed (in red) and wrote "For ATF Use only!" across the front, also in red. I want to make it as easy as possible for them to do the change. After six weeks, I had not gotten a new 03, so I called. They were very friendly and helpful and promised to get the amended one right out. That they did. Within days, I received FIVE copies of my new 03, each one identical to the others, and all mailed the same day and in different envelopes. |
