Every couple rounds fired an empty shell case will hit me in the forehead.  Is this a common problem with the firearm or could it be a problem on my end?  Other than this issue its a great shooter, feeds every .45ACP I've fed it.  This will be my concealed carry weapon once I figure out why the gun randomly ejects a shell case in the wrong direction.

Check extractor tension.

common with them, on the 1911forum there is a colt rep who fixes these, warranty will fix it for free, or, like I did, I took it to a local smith who tuned it

It is a common problem.  Mine was greatly reduced by several adjustments to extractor tension.  Surprisingly, my .45 Talo New Agent does not exhibit this ejection issue; so I've tried to get the same degree of tension on my Defender.

Mine did the same thing. I checked the extractor etc. I fixed mine with a little filing on the ejector. See my post on the same issue:

adjust/replace extractor, firing pin stop, install egw extra long ejector and shape as required for correct ejection patteren.
