I had some old, worn-out McCormick mags that similar things until I replaced the springs.

+1 on mags.

Is it one of the last rounds of the mag?   It appears that the round is popping out of the mag during recoil and the slide is coming forward on it.

What mag and how old is it?
Have you tried other mags?

Old mags.  Unless it's the lighting used, the first pic looks like there are pieces of brass in the pistol.

However this failure was after I replaced the recoil spring.

Could be a few things going on, but I agree with the mag spring issue, first off.  I also notice that your TRP is not quite stock, in that I mean, the factory trigger has been swapped out, the ambi safety removed for a strong side safety, also your slide stop(even though I cannot see the other side) appears to be after market by the way the nub is shaped and finish worn.  This is based on just the items I can see from the picture, not sure if anything alse has been swapped or "fitted" that may hinder performance.  All that being said, one can presume that your pistol may have one....to a few things going on.  Here is a pcture of mine that chugged on one reload at the range that fired off like a 9mm (maybe under-charged).  Anyhoo, outta the 300 rounds (200+ factory WWB and 100 reloads) or so that I have through mine, this is the ONLY malfunction I have had.  Mine is all stock with the exception of a fitted, dehorned and refinished Karl Lippard A3 extended slide stop that works/looks like a dream, btw.  Ejection of spent shells on my TRP AKote is also fantastic(8-rounds as seen in photo right to my right, darn near within reaching distance), drops em right by my side, everytime.  Hope your issues are resolved in short order, Partner!  I have never tried hollow points in mine either and use Wilson ETM 500's, FWIW.

I'm hoping it is the mags but, the mags are not crappy beat up knock offs.  Wilson ad Chip McCormick w/ Tripp followers are pretty nice.   The wilson mag is 3 yrs old and the Chip McCormick is prob 2 years old.  I think the flecks of metal in the pic were light anomaly's.

Very observant.  I have many upgrades on my TRP.

Between the 2 mags that failed I probably have approx 500 rds through them.  I wouldn't consider that excessive though.
