The pistol is a Springfield 1911.  I am experiencing failures to fully go into the battery.  It appears that the round in those cases is being fed in front of the extractor, rather than behind.  And there is not enough force to force the extractor to pop over the rim, preventing the slide from fully closing.  This happened about 4 times out of the last 100 rounds fired.  

Suggestions?

New magazines are in order, you may get away with new mag springs though.

The cartridge is popping out of the mag ahead of the slide, meaning the slide catches up to it but past the point where the extractor can get over the rim.

New mag springs, or new mags... and make sure feed lips are dry and oil-free.

in addition to the magazine issue...... radius the underside of the extractor, and it helps to lock in place with an oversize firing pin stop...if it's a .45,the extractor should have between 16 and 24oz of tension with a Weigand brass pull gauge...

maybe an 18.5 recoil spring, that would be the first thing I would do. thats just standard maintenance, but it can cause a lot of problems if worn out
