Hi,
I recently bought a used S&W 4006.  I really like the pistol except for for one thing.  Its not very accurate.  I took it to a friends ranch for some target practise and was all over the target and in some cases, out and out missed.   Dont get me wrong, Im at best a middle of the road marksmen however with my friends Witness I consistently got all rounds in the center of mass.   The 4006 has a 4" barrel and the witness had a 5"  but I dont think that could have accounted for the wildly different results.  
What could be wrong with the S&W?  Could the barrel be shot out?

Highly unlikely the barrel is "shot out" as they last for thousands of rds for the most part. S&W semis have never had a sterling rep for accuracy for their wondernines. Models 39, 59, etc. were ok but once S&W hit the big time with selection, accuracy seemed to take a huge dip.

You could replace the bbl with an aftermarket like Bar-Sto, they run about $125-175. I'd try to have someone check your bbl fit first. You may wish to call/e-mail S&W Performance Center & ask their opinion on options.

What bullet weight were you using?My experience with a 4006 was that it was very accurate with 180gr ball, but anything lighter, and the grouping got very erratic. You may also want to try various manufacturers of ammo, to find what it likes. Heavier seems to be better, though, when it comes to bullet weight.

My 4006 has a really crappy trigger that I think adds to the so-so accuracy. I shoot far better with my Glock 23 or USP40C.

Hard to tell what could be wrong with it from afar,  but the key word here is "used".
Must be some reason it was "offed"...a police trade, a home gunsmith's mistake or a just POS from new that was never sent back for repair under warranty.
If you really like the gun, except for accuracy, you may want to ship it off to Springfield Mass. for a once over, as one option.
A quick phone call to the factory may lead you to your next course of action.

I too have a 4006 that has less than stellar accuracy and I can't stand the trigger pull.  I have often thought about having the trigger worked on as that may be the biggest problem as far as I can tell.

When I compare it to the accuracy of my Glock 23 it's downright disappointing but since it was my first handgun it has a special place in my heart.
