Do the early series II pistol only have the firing pin safety?  Is the external extractor a later addition?  I ask because I saw a Custom II (I assume the II means series II) and it did not have the external extractor.  Just wanted to make sure I had the I vs. II differences straight.

You're correct. The early Series II guns had the FPS and internal extractors.
