Only had this gun for a few months and for the most part have been very happy with it.  I have noticed when chambering the first round out of the mag, it sometimes fails to feed.  The nose of the bullet lodges into the bottom of the feed ramp.

Is this something that will wear in over time?

Anyone else experience this?

I rack the slide hard and fast, I'm not easing it in there at all.

My only thought is make sure rounds are pushed back properly in mags so that they are seated all of the way back.   Rack slide all the way back and release just like you said.   May just be a stff mag issue that will loosen over time.

Make sure rack the slide with some authority. I've got two of these pistols and my wife was having the same issue when loading a fresh mag and using the slide release to chamber the fresh round.

Lock the slide back, load the magazine, use slide release.  Does the round have any feeding issues when doing it this way?

Any issues during firing with failures to feed?
