I sometimes have the same problem with my P220 and every Sig I fire.

I found that when I use a "Thumbs Forward" grip the slide doesn't always lock back on the last round (because the thumb of my firing hand touches or rests on the Slide Lock).

On most guns, that issue can be caused by a weak Mag Spring, bad Follower, or Recoil Spring.

But, in the case of Sigs, I would try crossing your thumbs and keep the thumb of the firing hand away from the Slide Lock. That should solve the problem. If it doesn't you might have to troubleshoot other causes such as the above components.

Hoe this helps.

Be Safe.

I had and still have this problem. What ishoot2live said is the exact truth for me.

I choke up too far, and my thumb keeps the slide release from coming up.

Modify your grip for a few magazines. It's my guess it will be 100% for that time.
