What can cause a an AK to double fire. I saw it at the range today and it happend twice,
Rifle was an 74 variant.

It's very possible the guy was bump firing from the shoulder.  I can do it on my 5.45 gun from the shoulder and 2 or 3 rounds out usng a regular tapco G2 trigger.  It does sound like a double or triple fire, and occasionally people will think the gun is an automatic or is doubling.  ARs seem even better for it.. less recoil makes the shoulder bump process easier to do.  

If it is really doubling, there is probably somthing wrong with the disconnector not working and allowing the hammer to follow the bolt home, resulting in occasional doubles.

I (apparently) over-polished a G2 once, before installing it, and it double tapped like crazy.

If the guy looked surprised he was probably not bump firing it. He needs to examine his FCG right away. It could be installed incorrectly or over polished. There have been some problems with single hook G2s and NDS-3 receivers. It is really hard to say withoul really knowing what kind of rifle and FCG he was using.
