I've put my rivet heads on the outside.  I use a gigantic bolt cutter rivetter because my medium sized one won't reach the middle rivet.

As for a how to...line up the rear rail hole to the rear trunnion hole in the receiver. Clamp the rail down parallell to the top of the receiver. Drill holes and rivet.

Your gigantic bolt cutter is a 24"er? I have one that I am putting on a 74' I have not had a chance to see the mount attached. Does the foward rivet and the rearward rivet need to be flush to the rail? Would a non-recessed rivet head get in the way of the mount?

I have had much better success with tapping the holes and screwing them tight from the inside, with a little locktite.
