Took my newly built ar to the range yesterday and first round fired right on mark, pull the trigger and nothing.Pull the charging handle back and there is a case in the chamber, had to push the case out with cleaning rod with not to much effort.
Loaded again same thing happened.Noticed rim sligthly torn bent from trying to be extracted.Used three other Pmags same deal case stuck in chamber, BCG closes on spent case, it is not chambering another round.
Mega upper M4, sabre defence 16" m4 chrome lined barrel 1x7, BCM BCG.
Got it home and loaded a few rounds and ejected rounds without a problem, rounds fit in to chamber nicely, as does a go gage.
Ammo used 55 grain federal, 62 grain Israeli SS109, and PP 69 grain match, all reacted the same.
Other than the M16 bolt, I am using a standard buffer and spring, with Vltor Imod stock.

Are you describing an intermittent fail to fire?  If so, check the orientation of the hammer spring.
Note that the legs of the hammer spring come from underneath and rest on top of the trigger pin.

Did you clean the upper/barrel to start with, including using a chamber brush by hand in the chamber to remove all the storage lube that may have shop debries in it as well???

Did you trying using a tube to blow down the gas tube to make sure that you have correct flow down the tube through the barrel bore, and not something misaligned along the way to cause a blockage of the gas system?

Did you check to confirm that the gas tube is correctly aligned with the carrier key by inserting the carrier without bolt in the upper receiver?

No I am describing a failure to eject. The first round fires, the empty case does not eject, and the BCG closes as though another round has been loaded but it has not.

Upper and barrel were cleaned, did not brush the chamber but it looks clean.
Gas tube is in right position and did blow down tube.Carrier is aligned to the gas tube.

When you first when to shoot the rifle, did you load only a single round into a mag, charges and fired the single round, with the empty mag still in the well, to confirm if the bolt was locking back on the bolt catch or not to see if at least the rifle was full stroking or not.

Also, at this point, need the configuration of the build (need to know all the parts and types, including the butt stock system) to try to trouble shoot the rig farther. If the top end is all right, then could be the wrong buffer or recoil spring in play, over even a bit of hammer wedging preventing the rig from correctly full stroking.

As for the bent rim on the spent case, could have just been for the after math cleaning, with before that, the rifle only semi stroking (not enough for the spent case to clear the action, and the now fire form expanded case just shoved right back into the system.
