After playing at the range was going to load the rifle back up to its ready to go status.  Not sure how this happened but the bullet got lodged between the charging handle and bolt with its nose stuck in the gas tube.  This was a reload from a locked open bolt by hitting the bolt release.

maybe you have a real tight mag spring? with alot of upload force? or a follower that isnt very stable? ive never seen that in my life.

At what angle were you holding the rifle when you hit the mag release? I could see that happening if you have a weak mag spring and hit the bolt release with the rifle tipped down at an angle.

The round instead of being stripped off the mag and guided up towards the breech, would simply fall down into the top of the receiver exactly where your photo shows it.

He was holding it 'gangsta'-style! That's one for the books - I've never seen a cartridge try to get into the gas tube.

Hartford mags, magpul followers and I was holding it level.  Hopefully just a one time fluke.  My mags are numbered so if it happens again with the same mag that would be a clue.

Good idea. I number all my mags, rifle and pistol. Most likely your problem was just a fluke.
