The forward assist is used to push the bolt into battery if needed.  It engages the notches on the right side of the bolt carrier and acts kind of like a ratchet.

Improper maintenance or riding the charging handle will do it.

The button is attached to a pawl that engages the serrations in the right side of the bolt carrier.  Each time you hit it, it pushes the bolt forward a little bit (if it's not already locked).  Gene Stoner said it was unnecessary.  Army Ordnance (which didn't like the M16 anyway) insisted on it, since both the M14 and M1 had a way to force the bolt closed.  IIRC USAF preferred the M16 as it was (without the FA) and the USMC didn't care.

It's discussed in The Black Rifle and there have been a bunch of threads on it.

Wasn't part of the reason so that the bolt could be put home silently in case that was necessary?
