Does anyone have a pic of the inside of one of these older carbine M203 handguards?

While I do not have a pic, what I would suggest is, since the M203 was so new (adopted in 1969) during the "retro" time frame  the unit armorers would most likely have used almost anything to get things mounted. I would look for a short nut and screw combo that does not extend much into the handguard since the gas tube is right there...




Remember the M203 was designed to mount on a standard rifle, not on a non-standard carbine in some SOF unit...

I have the handguard, leaf sight, and the standard M203 handguard screws will be long enough. I am thinking maybe there is a washer the diameter of the holes in the handguard so that the screws stay pretty well centered in them and then a flat washer and nut on the inside of the handguard.
