per the TM and manfs = yes

But I don't as long as I can get it enough torque on it.

this, I typically don't replace the washer if I re-install the same FH

I recently removed and reinstalled the flash suppressor on a 6920 without replacing the crush washer. It took quite a bit of torque to put back on, and I'm confident it won't come off. Colt also put some locktite on there that I did not re-apply.

That is what they're called, and you can buy them at many places. Have you bothered to check any of the site sponsors?

It's recommended, I'm an armorer so I always do it, plus they're cheap enough  (like a buck or two).  You can pick them up from most sellers, I usually pick a few of them up from Brownells when I am placing an order since it's not worth the shipping to just order the crush washer itself.

Most of the time yes... Twice I have not needed to changing muzzle devices and they still came out timed correctly. But 9 out of 10 times you do need a new crush washer to get the MD timed right...

I get all my crush washers from HERE. While I'm there I always stock up on small parts that I need to stock up my spare parts bin to offset the shipping costs. I use Windham, Spikes, ALG, and PSA "Premium Parts" make up the majority of the small parts bins with great results...

You can get them anywhere. Like another poster suggested, you should order them when placing another order so you don't get hit with shipping. Joe Bobs offers free shipping with orders over 89 bucks or something like that.

JoeBob dun saved you $0.55!

ETA: They ship super fast, probably the fastest I have experienced. Hell, your order has probably shipped already.....

ETA: No, I am not affiliated with them, they just always do me right.

There you go... I do this when I buy $50-$60 of small parts from Windham, JoeBobs, Primary Arms, Rainier, or my recent favorite, Aim-Surplus...
