I put it all back in and together and took one shot and it happend again what goin on with this thing, Glad spikes has lifetime warrenty this is my 1st spikes and I hope this is not how its gonna be from now on hope I did not get a lemon?????

contact Spikes, they'll fix it.  The receiver extension is probably is out of spec or not screwed in all the way.

Yep, sounds like an improperly installed receiver extension.

Put the spring in the hole that the little nipple goes in that holds buffer and spring

OK.
Stop shooting and send it in for repair. (or repair locally by a competent black rifle smith)
If you keep shooting, you could permanently damage the bore where the retainer and small spring live.

Sounds like buffer tube needs to go in one more turn. Pics would help though.
