Is it simply a case of bad ammo, that's it? Or can it also occur due to problems with the internals and the receiver?

It is simply the flawed design of the AR15 that causes them to frequently blow up.  I suggest you pack up your AR and all the ammo and send it to me for proper inspection.

The broader your definition of "explode", the more things can cause it.

If by "explode" you mean any violent event, some possibilites are:
- burst or bulged barrels can be caused by extreme overheating or bore obstructions
- case head separations can be caused by overpressure loads or excessive headspace
- blown primers can be caused by overpressure loads or very loose primer pockets
- pierced primers can be caused by overpressure loads or misshapen firing pins
- firing out of battery can be caused by misassembly of parts, high primers or jammed firing pins

I tend to think of "explode" as meaning "explode".  When this happens it is very distinctive, as the whole gun turns into pieces the size of a golf pencil.  This is caused by detonation of the powder, a phenomenon that is not at all understood.

None of these events are unique to the AR-15.  Any firearm will have similar events under similar conditions.

Why does the HK handle the water test so much better???

IIRC, it's because the gas system is different...

instead of a tube that transfers gas directly to the carrier, you have a piston that blows back into free space.

meaning there isnt really a sealed tube that can fill with water and block all the gas until it is excavated.

I am most likely missing quite a few things so someone with better knowledge should fill you in on the subject.

Isn't firing out of battery physically impossible due to the design?  The firing pin is too short to reach unless the bolt is fully pushed into the bolt carrier.

Simply put the HK416 is a far superior weapon system and is what the M4 should be. The HK416 also worked well in the dust test.
