I took my brothers new AR to the range today, It is a Superior lower with a DPMS lower parts kit, a complete A3 upper that we bought at a gun show, and the old Bolt carrier group and charging handle from my rifle.  I know you are not supposed to interchange bolt carrier assemblies but i just had this one laying around and he needed one for his gun.

I was at the range today firing this rifle for the first time, The first time i pulled the trigger it went bang, then when i tried to pull the trigger again nothing happened so i pulled the charging handle and a fresh unfired round came out of the chamber, How could this happen?  The bolt went back far enough to chamber a new round but it didn't  cock the hammer back?      Can some one please tell me why this might have happened.

sorry its so long.

Adam.

sounds like an out of spec trigger parts kit or the ammo was loaded light and the gun short stroked just enough to strip a round and hammer followed home.

my money is on the trigger parts

what in the trigger parts kit could cause this to happen?

It may not be the parts but the way they were installed.  Or by leaving out a very important small spring.

Remove the upper.  Take a look at the Fire Control Group, (FCG).  Make sure that the hammer spring legs are going the right way.  Also make sure the trigger spring legs are going the right way.

Then check the disconector and make sure the spring is installed in the tail of teh trigger.  Make sure the disconnector spring is installed right side up.  (Fat end down).

These are very easy things to install incorrectly or overlook when assembling the lower.  Don't ask how I know.

The disconnector or it's spring seem to account for the majority of the problems with the hammer resetting from what I have read here.  It'll at least give you a place to start looking.  Hope this is helpful.
