While installing a barrel on a new A3 receiver (older aluminum blocks)the barrel extension un-screwed.

I was under the impression the extensions were torqued @ 175 ft-lbs and the index pin went through the extension into the barrel.

Anyone ever came across this before?

No, and that sounds VERY bad!!!  Barrel extensions are normally so tight that they are virtually a permanent part of the barrel.

I WOULD NOT USE THAT BARREL!

It needs to be properly torqued, index and headspace need to be confirmed.

Better safe than sorry.

Retorgue it set it up get a go no-go guage and see if it works.
There was an article here how to make a barrel extension wrench. You do need a broken Bolt or one you are willing to cut and put into a socket.
