So we took my uncle out shooting for the first time yesterday. While he was shooting my 11.5" 5.56 SBR he was having the trigger go dead. As in, pull the trigger, round fires, gun cycles, go to pull the trigger again and its dead....(didn't reset).

I at first thought maybe the Silver Bear steel cased ammo wasn't cycling it enough (but If I'm right here... if the bolt goes back far enough to chamber a new round, it went far enough to reset the trigger). So I switched him to Federal .223, and he had the same issues.

This occurred on both my 10.5" .300 blackout upper and 11.5" 5.56 upper, the video shows the .300 blackout upper.

Is this something that can be caused by shooter error? I see lots of people claiming its the excessive back pressure and cyclic speed, but that doesn't sound right...

I picked up the rifle and tried to get it to happen to me and it always worked.... I would fire a few rounds, no issue... hand it back to my uncle and it would happen again.

The trigger is installed correctly....

It's most likely hammer follow which is similar to bump firing.  The rifle has to be held firmly, pulled into the shoulder and the trigger pulled directly.

Sometimes really good shooter have trouble with this as they put just enough force into the trigger to discharge, but no more.  The trigger finger floats and the recoild/counter recoil of the gun causes the trigger to be pulled again just before the bolt carrier is home

I had the same thing happen with the S3G in the AR I built for my dad. When I was zeroing it off a bench and being very careful with the trigger pull it failed to reset a couple of times. I haven't had the issue since though, so I'm pretty sure it was me.
