Hi all,

In a report writing class I'm taking, the instructor (who was valedictorian of the Ben Stein School of Public Speaking) wants us
to bring in a few blank incident reports from different agencies.

He wants to show a variety of report forms from different sources to explain the variables that you can run into.
Instead of going to a neighboring county, in true Arfcom fashion, I thought I might could get some from other states.

So, if any of you would happen to have a digital copy of any type of reports, I would be in great debt to you if you would
send me a  copy.

Introduction:

On 01-01-01 at 0001 hrs I Dep J Doe was dispatched to 1234 fake street in reference to males in the parking lot smoking weed.

Upon my arrival at 0006 hrs i located a black in color 2001 chevy impalla bearing Tx LP 123FAKE in the Southeast corner of the parking lot. Contact was made with the suspects who were found to be calm and cooperative, but intoxicated.

Scene description:

the scene is the parking lot of the apartment complex located at 1234 fake st. Fake st st is a two way public roadway traveling  east and west and is located in houston, harris county texas.


The suspect vehilce was found to be a black in color 2001 chevy impalla bearing tx lp 123fake. The Suspect 1 was found in the drivers seat with a lit marijuana cigar in this right hand. A marijuana grinding device which was composed of metal, is silver in color, and measured 3x4 inches cyndrically was found on the floorboard.


Officers actions:


Upon my arrival at xxx hrs i Dep J doe observed a black in color chevy impalla in the back southeast corner of the parking lot. the interior of the vehilce was observed to be enveloped in a thick gray smoke and the windows were partially rolled down with music playing softly. the engine was turned off but the running lights were engaged. Upon approaching the vehicle more closely the deputy observed the odor of burnt marijuana exiting the vehilce. the deputy approached the vehilce, flung open the door and while blading at 45 degrees and holding the suspect at gunpoint, he announced "freeze MF'er, im Deputy dog J doe, like judge dread, but more dreaded by stoners!!!" the supect was ordered from the vehilce and detained without incident. the deputy examined the cigar that was found in the suspects right hand and oberved that it contained a green leafy substance that the deputy believed from his exprience from previous narcotics arrests to be cannabis staiva l, commonly known as marijuana.  Assistand district attorney Jane doe accepted charges for possession of marijuana, less than 2 ounces. the suspect was placed into custody in the back of the deputies patrol vehilce and the vehicle was released to the suspects mother, Jane doe (w/f dob: 01-01-1981) with tx dl 12345678. the suspect was transported to xxxx pd jail where he was booked in an intoxicated state with a prisoner hold from xxx pd narcotics sgt john doe under hold card number 12345. the marijuana was weighed at xxxx pd narcotics room and found to weigh 1.23 grams.
the marijuaan was tagged as evidence to be tested and held for court and released to narcotics officer john doe with badge #`1234. charges were completed under DA log 1234.

sorry if that was hard to read. The post box has been randomly messing up lately and when the cursor goes down to a new line, every time you go to type a new letter, the box automatically jumps up to the next line of text, making it impossible to read the line that you're typing on as you're doing it.

Its really really frustrating.

its from some rap song thats really old. I was typing this at like 4 am after working an 8 hr shift that turned into 12 and it just popped into my head.

i dont see you spending 30 minutes typing your report format to help this kid. I put the "pithiness" in there to keep it interesting and amusing. He just needs to see the outline and how they're written, the specific quotes, spelling, and punctuation dont matter, i expect to clean the reports up before turning them in since its his school project, and obviously i dont put that stuff in a report. (unless a suspect actually said something like that to me, in which case i would DEFINATELY put it in the report".

Also, thats just the narrative. We also have fill in the blank screens that we have to fill out for suspect vehilce's, suspects, call summary, evidence ect.

When is the project due?

ETA:  It's sort of a shame I don't still have copies of our old forms that we had up until *2001* that still had "N" on them................................talk about being behind the times.

I talked to an HPD officer once that said he policed houston in the late 70's when all the reports were written in pencil by hand. He said the reports were completed on one sheed of notebook paper. The first 1/4th of the page had spaces for officers dispatched to the call, time of dispatch and arrival, call nature, and reportee info.

Under that was a 1/4 page to fill out suspect info or descriptions, then there was a few inches to list property taken in a burglary or such, and the bottom quarter of the page had about 4-5 lines to hand write your officers narrative (that big long thing that i did above. )

He said that a DWI report would say something like "officers observed susp veh speeding 65/55 and swerving out of lane. susp stopped, had soab (strong odor of alcoholic beverage upon breath) and rbe (red bloodshot eyes) stated he consumed a 12 pack, failed sobriety testing, susp arrested and veh. towed to impound at 123 baker st, susp booked.

Nowdays that report would run me at least 12 pages and take me 2 hrs to complete.

The next class is Monday night. Thanks everyone for the help.

Since HKUSP has given me a "story" to use, I now just need blank incident forms. Traffic, domestics, whatever. If there's a reg
with your department that you can't send me one, this "kid" understands.

It's no problem for me to get a local report form, but being my usual self, I want to overdo it. Just about everyone in class
will have a report form from either the local city or county department, I just wanted something a little more exotic lol.

In all seriousness just contact your local PD or sheriff and ask the crime prevention officer or someone that does that kind of thing and they'll probably hook you up.

I'm thinking of coding some software that would take a dictated report by the officer and convert it to an e-document. Eventually the paperwork will go digital just like every other industry - a software version could also allow dash cam video attachment, photos, recorded witness / perp statements and so on. If it was easy to use, quick and solid the time saved might make a huge difference.
