I was told over a phone to have a copy of my resume ready with a residence history on it.  Thats seems kind of weird to me... FWIW its for a retail sales position as a summer job for me between school.  How do I add it in / where wold I put it?  Also how much more do I really need?  I have my education and previous jobs on there, but should I add my extracurriculars such as lacrosse and fraternity involvement?

ETA:  Should I have all my addresses that I lived at down in school or just list my permanent HOME address?

That's weird. Stuff like that goes on the job application form, not the resume. Consider having a separate sheet with this information on it, if this particular job requires it. Also, it's helpful to have a separate "References" page.
