Surgery can increase length somewhat, by cutting the ligament that attaches at the base of your penis and, essentially, "pulling more of it out of your body". Your penis extends pretty far inside you, with only about half of it (give or take) being on the outside. Surgery can move an additional percentage outside.
Of course, there are risks. Imagine cutting the roots off a tree: the trunk may still be firm, but it will fall over. The same with this surgery; you may be longer, but you may not extend straight out from your body. And there are always risks of infection and other complications, as with any surgery.
Ultimately, the medical community agrees that surgery is the only real way to add length. All of those pills are total crap, as are things like vacuum pumps (which can even damage your tissues!).
IMO, you'd have to have some pretty serious "shortcomings" to risk surgery.
Keep in mind that various studies have put the AVERAGE length at anywhere between 5" and 6", with "normal" being in the 4"-7" range.
I think porn has a lot more guys concerned these days. While breast size in porn may be an issue for women with all of the breast enhancements going on, there are still plenty of natural, smaller-breasted women in porn. The MEN in porn are a different story; they are virtually ALL in the top .5% in size, and give a very skewed view of what is normal.
Whatever you do, be careful with your wang; if you mess yours up, you don't get another one.
