One section asks for "Travel Plans."  Well, I have none.  But, I just want to get a passport "just in case."

Just leave it blank?

Furthermore, they ask for information about your, in my case, former spouse.  I think I got her birthday right, but not absolutely sure.  Is this a big deal?  No way in hell I want to contact her to find out.

Just put 'No Immediate Plans' for the travel portion.

Just tell the clerks you want a passport and they help you fill in any slots.  Most of the questions are just BS they don't really care about.  The spouse thing is probably just a "In case of emergency" thing.  They don't check up.

ETA: Just make sure you have a certified copy of your Birth Certificate
