For the "proof of citizenship" it says to include a copy of a birth cirtificate or copy of a passport. I can't find a birth certificate of mine and my passport expired last year - can I still use it as proof, or is there something else I can use?

I'm pretty sure that an expired passport would be fine, given that it's not your citizenship that's expiring. I'd send it in and if they have a problem with it they'll call.

you should be able to get a copy of your birth certificate from the courthouse in the county in which you were born.

A copy of the information page of an expired passport is fine. I've used one several times with CMP.
