How yall doin?  I've have a problem that goes off and on for the past 6 months.  Occasionally my right testicle is in pain (feels like a swift kick in the groin), usually one day every three to four weeks.  If I go to sleep, when I usually wake up it's gone and I'm fine for the next three to four weeks.  The pain is always located in the right testicle.  I do monthly checks for bumps or enlargments, and everything feels normal.  One guy at work mentioned that a buddy of his had the same condition and turned out to be cancer.  Some physical information I'm 24 soon to be 25 yrs old, 6' 3" and slightly overweight (250 lbs working on getting it down).  Bad Habits: Smoking working on quitting down to 3/4 pack a day,  I live off of coffee I usually drink 1-2 52oz cups of black dark roast a day.  I rarely drink (3/4 of a case of beer left over from 4th of july).  I'm eating at home instead of out (Atkins).  I usually wear loose fitting pants and boxers.  No problems with urination or bowel movments.  Guys, I don't know whats going on, and I'm a little worried.  I won't be able to see a doctor till after the first (insurance).

Time to break out the Leatherman or some other sharp implement.
Isolate the testical in question by tying a string around your nutsack just above the little bugger. Monofilament line [fishing line] works very well for this. Make sure your blade is very sharp and cut through the nutsack just above the nut and below the knot in the line. Take the now severed nut and feed it to your cat. Pour whiskey on your balls, er I mean ball, to kill the pain. Drink plenty of it too.

I also forgot to add in Aug of '02 I was hit while walking accross the road by a noncompany van doing approx 20-25mph while at work.  Since then my shoulder and hip on my left side have been in pain and workmans comp has failed to fully rectify the problem.  Their opinion is if you can walk you can work.  I know I shoulda sued the hell outta that guy and the company that owns the van, but I was too nice of a guy (read dumb@$$) to call a lawyer and file a suit.

Well, first of all you need to see a physician as an internet diagnosis will hardly suffice.

Do you masturbate a lot? You could have a condition known as epididymitis- a painful inflammation of the tube that carries semen to the urethra.

Why do you associate choking your chicken with  epididymitis,  It has long been thought that straining or strainful work was a contributory factor in epididymitis. Recently, however, this has been considered to be less of a factor and infection is now considered the most common cause of epididymitis. The infective agents most likely to cause epididymitis are the bacteria which commonly cause other urinary infections. A much less common cause of epididymitis is the gonorrhea bacteria, but I see no mention of excessive masturbation causing this condition.

I'm gunna say go see a Dr.  A close friend of mine had the same problem. Pain in the left testie. It was cancer, he had it removed, he is currently undergoing aggressive kemo and radation. He is schedualed to undergo removal of several lymph nodes on the 17th. He's thankful it didn't spread to the prostate.

I'm glad he caught it in time. oh and he's 24.

testicular strangulation (testicular torsion) or cancer would be the most serious causes though the intermittent periods of pain makes me doubt those. Still, there are rare cases of testicular torsion which have recurrent periods of pain. Usually happens in guys a little younger than you. Could be a hernia, epididymitis, or acute orchitis. I would venture a guess of some type of infection but the other causes are serious enough that I would go to the doctors to get them ruled out.

Testicular cancer is more prevaient in younger men.  You are well within the demographic.

Get to a Dr. fast because testicular cancer is a nasty, very fast growing kind.  The sooner you catch it the better.

Of course it could be anything but get checked anyway.  Fast!

I am a doctor.

Go see a doctor. Get a testicular ultrasound. Cancer question will be ultimately answered by that test.

Epididymitis causes pain in the struture above the testicle, rarely in the testicle itself.

Testicular has a very good cure rate, even in advanced stages, but earlier detection is clearly the better option.

Finally, the most likely problem that you have is the one of the banes of a urologist existence (I'm not a urologist). They simply call it testalgia (pain the testicle). Usually, going from boxers to briefs, or my favorite boxer briefs, will cure the problem. But testalgia is a diagnosis of exclusion. Get the ultrasound first.

I had a similar problem - it was the result of an athletic injury (tore an muscle while stretching, and it bonded to something it shouldn't have when it healed). I had to go to SEVEN doctors before I finally found a urologist that seemed interested in fixing the problem. Correction was surgical & totally effective, but not that bad (three day stay in hospital, six inch scar, cute nurses).

Go see a doctor, then if you aren't 100% convinced with the diagnosis, go see another, and another, etc.

After you have made a doctor's appointment, do the following tests/inspections:

1) Does one nut seem too high? Normally the little fellas move up and down, but if one seems kinda stuck a bit higher than usual, it may be a hint.

2) Check to see if it hurts when you are cold. You'll need to crank up the air conditioning and walk around in your boxers for a little while, or if you are in a northern area, just go outside for a while. This should cause your nuts to pull up. If this causes pain, again it's a hint.

This won't help YOU diagnose the problem, but it might help your doctor to figure out what's wrong.

I have the same problem ocassionally. I am 19, healthy, with no smoking or drinking habits. Everytime the pain comes, I consider going to the DR, but then the pain goes away and I just leave it alone....

dude..

i'm a registered medical technologist and 2 summers ago i had a similar experience.

it turned out to be a epididmydis (Sp) but i do see SEVERAL men (ages 25-45) undergoing testicular cancer treatments and or testicular removal due to tumors.

get to a DR..

i fired my Dr over his handling of my case. he basically came into the room with a script for anoxicilllan and said" this ought to fix your up" ...with NO EXAM..

i asked if he was going to examine me to check out for cancer and he said no..

i told..him i wasnt leaving till i got checked out for a tumor..

"he could go along with the plan..or scream for help prison style..but he was gonna check my boys out."

i checked them out but bitched about it the whole time..

i've sinced found another DR..

DO not..

REPEAT..DO NOT..blow this off.

have it checked out.

it could be something that antibiotics clear up..it could be something more serious.


DO IT FOR THE "BOYS".

Shit, I'm back....

Had a similar problem. I didn't wear underwear for 30 years, and the boys hung pretty low....

I got similar pain to yours. Started wearing briefs to support the boys, problem went away...

(I ALSO, had an ultra-sound, and am post-op left hydrocele, and vasectomy...

I'm outta here....

I had the same problem for years. I was treated for epididymitis on many occasions with antibiotics. I kept going back to the Doctors and complaining of the pain and finally had a testicular ultrasound which was a story for another day. They found a mass and I had surgery, the mass was not cancer but they removed with my prior ok, the epididymas from both sides. I was told that would cure the problem of the reoccurring pain, Wrong, I still have the same problem just not as severe.

They did a major workup on me and found enlarged lymph nodes in my chest. After a lymph node biopsy they think I have been fighting some type of infection. The lymph nodes were negative for cancer.

So I would encourage you to see an urologist even if it turns out to be something minor.

I went throught the same series of on-again off-again pain in my left testicle. Long story short, went to Dr., he juggled the boys, took some blood, did and untrasound, told me I had 6 months to live....at the outside. Then the results of more tests came in and he said to forget about the 6 month thing as the CA had not spread. Went into surgery, came out without one of my boys, got a clean bill of health 6 weeks later. Whew.

During the middle of this episode, I studied up and found that 70% of males will have some form of testicular cancer in their lifetime. That's right, 70% (seven of ten)[:O], and they fall into two rough categories, first are the young men 18-35 or so and the CA is deadly, think Lance Armstrong, it moves to your lungs and brains ASAP. The second group is really old guys 70+ and the CA is much slower and will not usually cause any additional problems.

Bottom line........

First, go see a physician ASAP, as most of the above have suggested!  All of the causes listed above are at the top of the list for testiculagia / testalgia in your age group.  There are several questions that must be asked and answered (when did it start, what were you doing when it started, how long has it been present, is it on/off, more on/more off, what type of pain(sharp, dull, achy), associated signs and symptoms, what makes it better, what makes it worse, have you ever had this pain before, if so when, medications, allergies, travel experience, medical history, surgical history, family medical history,  social history{type of work, sex partner contacts}, etc).  Some of these questions you answered and some you probably do not want to answer here!

Some causes from above:
Inguinal hernia- Is a rather common cause, got to get to the surgeon so he/she can heal with steel.

Infection (epididymitis)- Antibiotics for infection, may take up to two weeks to see any improvement.  May be seen on ultrasound.

Inflammation- Anti-inflammatory (naproxen) for the inflame., can again take one to two weeks to see any inprovement.

Testicular cancer- as TWIRE stated, same with surgeon.

Torsion- as above and surgeon

Hydrocele / Varicocele / Spermatocele - Hydrocele is a fluid collection surrounding the testicle. Varicocele is dilated veins in the scrotum around the testicle. Spermatocele is an out pocketing of the epididymis that is filled with sperm. Spermatocele may be seen on ultrasound. Variocele may be felt when you strain as to have a bm.

Gallbladder disease- YES, gallbladder disease.  The gallbladder is always a possible source of pain no matter where the pain, I believe!  Had two guys both with testicular pain as mention in this thread.  Worked the first up over a two week period, finally got a ultrasound of the gallbladder.  Positive for gallbladder disease so off to the surgeon he went.  Second guy was diagnosed much faster as the gallbladder is now part of my work up for testicular pain, if I am suspicious and there is pain associated with food intake.

And more uncommon causes

No matter get to your primary care physician.  Get the antibiotics (doxycycline, amoxicillin, rocephin, cipro, avelox just to mention some of the more common antibiotics), schedule and complete the testicular ultrasound, and follow up with specialists per your primary care provider.

Perhaps you have a kidney stone. That would explain the intermittant nature of the testicular pain. It would seem to fit the pattern along with the history of lots of coffee,(which is actually dehydrating you).
A urine dip for blood
would also help support the diagnosis. I agree with the testicular ultra sound, but would get a IVP as well.

First of all I've seen every episode of Emergency, Rescue 911, Scrubs and ER so I know what I am talking about.

GO SEE A DOCTOR TODAY!!!  Sorry to say this but waiting until the 1st for your med insurance to take effect might be too late.  Just ask my friend, Joe "Lack Sac" Pierson, what can happen if you wait too long.

It doesn't take 6 months to pass a kidney stone.  

Do you perform a testicular self-examination regularly?  Notice any new bumps/lumps or new growths lately?

Could be scar tissue from a previous hernia correction hanging up or snagging things. (Don't ask how I know)

However, that is the least of all the nasty probabilities.  Go to a Doc quick.  If you wait too long the guys here will start a beeting pool on what it is.  And if you don't get a professional answer, they send Guido down to find out with his little friend.

I am a medical student. I am not a doctor.

That said, the RIGHT testicle (don't ask me why) is commonly a site of pain when you have a kidney stone either in the kidney itself or in the ureter (tube from kidney to bladder).  That may explain the intermittent pain.

The right testicle is also a site of referred pain in appendicitis.

All the other suggestions are possibilities, including testicular cancer, which is most common in younger men.

GO SEE A PHYSICIAN IMMEDIATELY, and, this is the most important part -- if he does not give you a thorough interview ("history") and completely examine you thoroughly ("physical") FIRE HIM and go to someone who cares about your health and not getting another billable visit in the office.
