I work next to the state skydiving saftey instructor and he was telling me a story that matches this exactly... they were all looking at each other and high fiving and then one checked his altimeter and got wide eyed and deployed silk.  Then everyone else immediately after.  They all almost died.  He sayd when you are getting close to the ground, the picture of the ground starts changing really fast... thats when you know you are really low.

Loss of situational awareness.  So wrapped up in RW that the had no clue how low they were.  Without an AAD they'd be dead.
