I work in the oil field. near misses scare the crap out of me.

Over time, it has been proven that for every so many near misses that happen, there is usually one death that occurs.
If you start to see this happening often with no change in safety management, expect the inevitable.
Learn from the mistakes that made it happen and implement safeguards to try to ensure it doesn't again.

Many years ago my dad was driving his (well-used) Fiat 124 Sport Coupe. He turned into a restaurant driveway, heard a "clunk" noise, and his steering went away.

Turns out that one of the links (IIRC the center link) snapped! His front wheels went in separate directions. If it had happened just ten seconds earlier he'd been SOL, in traffic on a major street in Los Angeles.
