My brother in-law ran a really large trucking outfit here in jersey and about 15 years ago they had to call some guys wife to get down to the warehouse as fast as posible because when they lifted the forklift off his midsection he would be dead in an instant the paramedics said.

He was driving the forklift and he got to close to the edge of the loading dock for some reason and the forklift went over, the operators are told always to stay in the protective cage even if it flips over, he tried to bail and got crushed between the dock and the large forklift, true to the para medics word as soon as they moved the lift off of him he was gone.
the funny (strange thing is that my brother in-law told me the guy was in no pain while the lift had him pinned, he said as soon as they moved it he let out this blood curdling scream and then just went dead limp and died.
