Truck fell off the hoist.  That happened to a customer's truck once at our shop. It was my day off but I showed up to check my other days off, and apparently it had just happened when I got there. The truck was laying on it's side and the hoist still about 6 feet in the air.

Had a boss of all people leave a Bronco in the air overnight once.
Came in the next morning and it was on its nose with its ass still hung up on the lift in the air.

A similar thing happened at a shop where I worked. A guy took in a friend's truck for  some transmission work, left it on the lift over the weekend. He left some jack stands like this under it:



Over the weekend, the compressor serving the  hydraulic lift shut off, the pressure went down, and the lift retracted. The truck, which was now only being held up by the jack stands, fell about 6' onto its side.

eta He was supposed to have had a paying customer on the lift. He packed his tools.
