Loose bolt I think.  I tried to search for something and asked my BIL who works in the aviation accident business, but all he could find was a description about a bolt being loose or shearing causing the centrifugal forces to to start tearing it up.  Not sure if this is accurate though.

It was that extra screw after assembly they had and didn't know where it went.

Ground resonance.  It will tear the shit out of a copter.  From what I have been told the only way to stop it is to power back into the air.

The helicopter on MacGuyvers show intro actually goes into a resonance fit and the door flies open.
