How do you raise your HDL numbers, 58 years old, doc keeps on my ass about raising HDL, nothing seems to work for me. I don't take any form of meds. Is there a sure way?

Niacin,  but it can't be the no flush type.

Of course exercise and proper eating too.

I'm 58 and according to my doc, my HDL levels are very very good.
I attribute that to aerobic exercise, I take my dog on a four mile walk every morning.

This works.  It's not very comfortable the first couple of time you take it but, it does work.  Here's a pretty good article about taking niacin.

I keep thinking the whole scheme is rigged.

"No, your LDL levels look good, cholesterol is ok, but OMGFBBQ!!! your HDL's are too low!!!! you're gonna fucking die!!!!"

Ok, so the doc didn't say it that way, but that's how I feel.

My doc says the same thing, I am stuck at 42 HDL

Coconut oil and other saturated fats will raise your HDL while maintaining a favorable ratio. Many of the things we "knew" about cholesterol are turning out to be false. If you have the time, please read this series:
