I've heard some people put grease on the lense to avoid any damage from the muzzle blast. Any truth to that? What do you guys do if anything?

That sounds like it would make it worse to me.

I don't have any trouble with the lens on my TLR1 on a Glock 22.   I make sure to wipe it off every so often when I'm putting a bunch of rounds through it.   It still wipes clean after six or so years... and I bought it used.

What would be the point of grease? Need something to trap all the residue on the flashlight lens?

Never had any problem with the lenses on my x300s riding on G17s, G19s, or M&P9s as well as the one that rode all the way forward on the rail of my AR.

Might be a problem if you were running really dirty ammo or dirty guns like AKs

Never had an issue with any surefire I own.

Our dept. allows the use of TLR-1s, and I haven't seen any issues with frosting or crazing due to muzzle blast.  I have seen a lens that wasn't properly cleaned, after many rounds, that had some stubborn carbon buildup.  However, a shot of MPro7 took care of that without any issue.

Chapstick allows easy cleaning of the lenses post firing.
