I saw Slayer and Pantera together sometime around 1999.  Not during either band's "prime" but both bands put on amazing shows.  Phil came out to sing with Slayer and Kerry King played a few songs with Pantera.  One of the best concerts of my life.

Correct.  I knew it was sometime in high school.  Thought it was earlier for some reason.  Didn't care much for Static-X, but Slayer/Pantera/Morbid Angel was awesome.
