I found the culprit.

It was this bastard:








Happened over 177 rounds of shooting.  Functioned flawlessly, and although it has hammer bite like a son of a bitch, it shoots nicely.
It shoots a little high, but it is pretty accurate for a bone stock 1911, IMO.
