Over the past few weeks, I have started to experience some pain in or near the lateral head of my left triceps.  I initially noticed it when doing pushups after lifting (my employer tests us on pushups, so I try to throw them in now and then).  I have cut out the pushups, but still notice it here and there.  The pain isn't constant, but tends to manifest when locking out on bench, or just after the bar passes my head on OHP.  Interestingly, with the bench, it doesn't seem to really affect me on my work sets.  It usually starts during warmup, once I hit 185 or 225 lbs.  I will also feel it a bit the night after, when I bend my arm, stretching the muscle.  The pain doesn't affect my elbow, and it doesn't cause any shooting pain or neural compression.  There is sometimes minor pain when palped, but nothing extreme (which makes it sort of difficult to really pinpoint the exact center of the pain).  Actually, sometimes it seems to almost come from underneath the muscle.

It is sort of hard to really pinpoint where the pain is coming from, but the ellipse in the picture illustrates the general location.  It seems to be right at the tip of the large part of the lateral head, and into the smaller extension (I don't know the exact terminology for that part of the muscle).   It seems like more of an issue with the muscle than with the tendon.

Anyhow, has anyone else experienced this?  Does anyone have any input as to what I should do about the problem?  Is it likely something that I should just bear for the time being, or should I rest?  Should I be trying heat or cold therapy?

I usually avoid giving advice to questions like this beyond "Go see a doctor!", however I think spending the money on an massage therapist might be a good idea in your case. I think that would be step 1 or 2 for me if I was in your shoes.

hold your arm out and up as if being sworn in. Have a friend or spouse hold your elbow and push your hand back as if slapping it. See if the pain gets worse, might be a bone spur hitting the rotator cuff ligament . had a friend with very similar issues, went to the doctor and they did this simple test, along with x-ray to confirm. But he said he felt the pain mainly in his tricept and down into the forearm.

he did the cortisone injection into the joint to help with the pain etc. ligament was not torn. But the bone spur was hitting it causing inflammation putting pressure on a nerve.

I am no doctor, but just last 2 weeks ago my friend had the issue. So guess its just a thought.

on edit-- he said the pain would start when doing work , but over the weekend he took it easy and the pain would go away. As soon as he started doing manual labor within a day it would act up again. Said it felt like constant muscle cramp. got real bad at one paint he thought he was about to have a heart attack. pain was mostly in the area you have in the pic.

Could be referral pain from shoulder impingement or from the elbow jt.  Hard to say exactly.  I get similar pain when my elbow jt is out of alignment or my neck/upper back.

I am wondering about referred pain.  I haven't lifted in a while, due to hand pain.  My shooting and music is a little more important to me, and both have been negatively impacted.  I have some sort of impingement that is affecting at the least, my ulnar nerve (or so it seems).  Do you mind if I send you a PM?
