I wanted to get something clear. When you insert the magazine you are then hitting the bolt release on the left side of the rifle? Or are you expecting the bolt to release when you insert the magazine? Also, have you tried pulling the charging handle rearward after the mag is inserted to see if that will release the BHO?
Did you go through the adjustment procedure with the BHOA? Try that again, I had to do it two or three times to get it perfect. Also, examine all the parts and make sure all surfaces are smooth and don't have a small burr or a sharp edge that might catch and hold on something.
I have not head of the exact same problem you are experiencing which indicates its not something that is common. So, we have to look at something specific to your set-up. I have a couple of these units and they all run very smoothly and trouble free. (But they are well broken in too).
