It takes less than a minute...regular contact info and some yes or no questions...don't do like my buddy and answer yes to the "are you mentally incompetent" question.

unless you are actually mentally incompotant...how do you say that you made a mistake on checking yes for mentally incompotent without looking mentally incompotent?
