Cowboom.com
It's backed by Best Buy and most have a warrantee and extended Warrantee can be purchased. You can buy the phone at price listed or use the auction feature. The product descriptions are pretty accurate and will let you know if something is missing.

I have bought several phones from them, they grade their phones pretty well, and most of the time you will see a picture of the exact phone you are buying.
mobilekarma.com is another outfit I have bought phones from with good results, they have better prices, some times, and tend to grade their phones more conservatively. A while back I ordered a Motorola Devour for my sister, it was graded fair, which by their definition is pretty well used, when I got it it could have passed for new.
ETA: Both of these sites sell phones that have had the SN's verified by the carriers to be valid for use. You don't sign any agreements or contracts when buying a phone from them and I have had no problem at all adding phones bought from them to contract plans with Verizon and T-Mobile, I have also activated phones from these sites with various prepaid carriers with no problem.
