Some good answers so far....

You may also want to verify the bullets you are using if you are reloading. If the bullets are a bit undersize they may not be engaging rifling.

See if you can recover a fired bullet. Compare it to other fired bullets that shoot accurately.

ETA -
KeithJ in the other thread has a good point. A damaged crown will SERIOUSLY mess up accuracy.

Do the bullets key all the time or just part of the time? If it's only with a certain manufacturer then the bullets may be undersized and/or not engaging the rifling properly. It could also be that the bullets aren't traveling at a consistent velocity and the twist rate isn't fast enough for them at that speed. Need more info on loads though to get a better idea.

Well.....how does the bore and crown look? Do you see anything unusual?

Have you done anything unusual to the gun prior to the key-holing?

Has it always key-holded since new, or did it just start happening? If it all of a sudden started key-holing, what changed with the ammo or gun prior to this occurence?

it was several rounds from same 100 round box of winchester white box .45acp

One of my co-workers loves cheap pistols, like Davis, Bryco, etc. and shoots STEEL bullets through them.

Enough key-holes to make a locksmith horny.

Sometimes the rounded .45 projectile will tear paper instead of punching a clean circular hole through.  The torn hole looks a lot like a key-hole.  Be sure that isn't happening before you freak out.

i will have to look at it when i get home tonight.

looked ok when i was cleaning it last night,didnt really check the crown though.
dont think there is any damage to the pistol,at least nothing jumped out.

it is the wife's pistol and has been pampered.

Well then, that almost makes it a certainty.  Even 9mm will do that (only FMJ) if the paper is just hanging from the top.

I've seen one gun keyhole due to incomplete rifling of the barrel.

A Walther .22, as I recall.

Came like that new out of the box.

There's no way any modern .45 will keyhole, that's just crazy.

That is unless they forgot to rifle the barrel.
