Depends. If it's someone you don't know at all, I like to start off with coffee. Very low pressure, she's unlikely to flake, and gives you a cheap out if she's inflicted with the boyfriend diseases. Then I like to go to a bar/tapas/sushi place for no. 2 - not a full sit down fancy resturant, but you get the idea, somewhere cool. Then let it flow from there. Cooking her dinner is a GREAT idea. If you don't know how to cook, learn.
ETA: Coffee also covers you for non-drinkers and to a lesser extent, Mormons.

When I was younger and less self-confident, I freaked out and didn't follow up with girls whose numbers I'd got, etc. etc., because I couldn't think of a decent date idea. If she likes you, she'll probably enjoy anything, and if she doesn't, well, no amount of crazy fun maddness will change her mind. The great man Swingset once said: you cannot force a woman to like you. Your actions (with obvious exceptions for BSC stuff) are not going to change her feelings on you. Quoted:

Eat lunch some where, I dislike coffee so no coffee shops for me. Eat some where the is is quick so if don't work out you get the hell out of dodge. Like a Mexican grill type place as an example.
Also shows always seem to work well for me, like plays, opera, or comedy. |

Ones I've used:
Afternoon coffee/drinks that can turn into dinner. Sneak into SeaWorld. Go on a tour off the USS Midway (replace with local equivalent). "oh, what's that nextdoor? A fancy-as-fuck restaurant? Drinks at sunset overlooking the bay?" Pistol range with .22s. Posted Via AR15.Com Mobile |

| Be different than all the rest of the guys and take her to do something that she'll think is really fun. Try a dance class or cooking class. It works great and will take the pressure off you from keeping her entertained with stimulating conversation for 2 or 3 hours. |
