I am sending in 2 form 4s at once.  I know I need 2 copies of the form 4 for each and a total of 4 fingerprint cards.  But do I need 4 copies of the certificate of compliance?  Can I write one check for $400 or do I need to write 2 checks?  I have never sent in multiple forms at once before.

I always send them seperate.  Don't want anything getting messed up, no real reason to send it all together.

Maybe I don't understand their process.  I was assuming that if I sent in 2 forms that the same examiner would send the prints to the FBI and do the processing for both forms at the same time so they  would get approved at the same time.  It would seem like a waste of time to do the whole process twice by separate examiners virtually at the same time.

I've sent in six forms at once. The way I do it when I send more then one form is:

2 finger print cards
1 cert of comp

Paper clip those two together

2 copies of the form 4
1 check for that form

Paper clip those two together

Repeat for the checks and form 4's for the number of them I have. I've yet to have any issues doing it this way. The finger print cards get sent off to the FBI, the forms that were in the packet with those finger print cards are marked that they went with those cards. When the finger print cards come back, they all get put threw at the same time. I've heard that you can send one check for the total number of forms, but I like to just be sure I have them all covered, and checks are cheap.

That's correct.

I would strongly advise separate checks. I read on here several people having trouble with just sending one check. Others have had it work fine, but I'm of the opinion that it takes long enough as it is, without risking anything that would lengthen the process.

I've sent upwards of five Form 4's at once (3 cans, 1 SBS, 1 AOW).  I sent in a total of 10 Form 4's, 2 fingerprint cards, one certificate of compliance, and one check for $805.  It all went through without a hassle.

Correct. Mine have always gone threw with one set (2) finger print cards, and one cert of comp. I've heard of other people say that they had issues with only sending one check, so I just send in a check for each item. Why chance that?
