I am filing my form 1 under my LLC.  In question 6 it asks for Special (occupational) Tax Status... In 6a it asks for Employment Identification Number; do I include my FEIN or do I leave it blank since I am not claiming a Special Tax Status?

Leave it blank.  That field is for SOT (NFA) dealers/manufacturers.
