What all paperwork do you have to send in, if going the LLC route?

Trying to help my brother-n-law out with a Form 1 with his LLC.

Duplicate Form 4's - no CLEO signature or photo needed.
Certificate of Compliance
$200 check
Copy of the LLC paperwork - I send everything.

+1 on what 1952 said. That is what I have sent in on all of my form1's and have had NO problems. The shortest Form1 I got was four years ago at three weeks mailbox to mailbox and the longest was a Form1 I got approved last month which took 9 weeks mailbox to mailbox.

Ain't LLC's great???

I sent my copy of my certificate of formation, stamped by the PA Dept. of State.

I did not send a copy of my operating agreement.

Worked for me ... two Form 4s approved, one more to go!
