The manual says that my gun should have included a cylinder choke (no notches) and an improved cylinder choke (4 notches). I received:

1. 3 notch choke that says lead shot Modified / steel shot Full
2. 4 notch choke that says lead shot improved cylinder / steel shot Modified

What did your gun come with?

Slugs - is it okay to shoot a 2 3/4" lead rifled slug through the choke with 4 notches? I am a bit concerned that the 4 notch choke says "modified". I don't want to damage the gun.

you can shoot slugs through a mod choke however your accuracy will probably not be as good.  if you are looking to purchase more chokes for the gun it takes browning invector plus tubes.

The FN SLP use Browning Invector Chokes, not the Invector Plus.

My FN SLP came with the same chokes as yours.  

Slugs are intended to be shot with the Improved Cylinder choke.  In fact I can fire slugs with the Cyl, Skeet and Imp Cyl tubes with no notable accuracy issues at all.  You want an open bore to shot slugs.  If you use other chokes (that close the bore muzzle) the slug will lose accuracy at the moment it leaves the bbl.  In the other hand, when you shoot bird or buck shot, you want to close a bit the muzzle to get the desired pattern at the intended target distance.  

You can get extra chokes (Browning Invector) for your shotgun.  I prefer Briley X2 Chokes (extended, so you can install/remove them with your hand... no tool needed).
