Unfortunately it is marked down for me to pay $1000 and not the reduced rate of $500. 

There is however a box that you can mark under Eligibility for Reduced Rates.  I wonder if I just send the form in with that marked and my $500 if everything will workout fine.

I would just print out another form but, the form itself is hard to find online and its all dated to next terms dates.  I suppose I could call or email someone in the NFA branch about it as well.

Item 3 on the back says - Check YES only if you are a NFA manufacturer or importer (I am a mfg) and your gross receipts are less than $500,000 per year for your entire business entity.  You must declare your eligibility each year.  The regular rates were used to compute the amount shown on this form.

Every 07 gets the $1000 bill.
If you follow the instructions (checking "yes" on the reduced rate) and send in a check for $500..........you are fine.
